Good health declaration
A signed statement of health that replaces a questionnaire on simplified products.
Some simplified products carry no medical questionnaire. Instead the applicant signs a declaration affirming they are in good health and have not had specified recent events — an extended hospitalisation, or a long absence from work through illness.
If you cannot sign it truthfully, you are not being refused; you are on the wrong product, and belong on one that is medically underwritten.
